Incident Reporting & Escalation

Everyone is responsible for raising concerns. This procedure explains how to record safeguarding, health and safety, or conduct incidents and how they are triaged by the Designated Safeguarding Lead and Directors.

When to report

  • Any safeguarding concern or disclosure involving a child, young person, or adult at risk.
  • Safety events including near-misses, injuries, equipment failures, or environmental hazards.
  • Breaches of the code of conduct, data protection, equality, or anti-bullying standards.

How to submit

Complete the incident form and notify the Designated Safeguarding Lead within 24 hours. If immediate danger exists, contact emergency services first, then inform the DSL and a Director. Preserve evidence and avoid internal investigation until instructed.

Recording & follow-up

Incidents are logged, risk-assessed, and reviewed for patterns. Actions may include statutory referrals, contacting parents/guardians, equipment quarantine, or temporary suspension from activities. Outcomes and lessons learned feed into risk assessments and training.
